Choose Between Two Benefit Programs

Wellness Option

The Wellness Option encourages members to make a personal investment in their health and take the actions necessary for health improvement by rewarding them with lower fees and copayments.

Wellness Option participants have the following responsibilities:

  • Complete a Health Risk Appraisal.
  • Select one or two health goals to work on over the next year.
  • Meet with a Nurse Care Manager within one month of signing up for the Wellness Option to establish a plan to achieve health goals.
  • Engage in at least two healthy activities or health related classes within 10 months of enrollment.
  • Participate in classes related to the participant's chronic health conditions or diseases, such as diabetes, high blood pressure or obesity, and meet with a Care Manager reg arding their management.

If requirements are not met, members are transferred to the Standard Option and will no longer receive the lower rates. Members may change plans during open enrollment periods designated by HealthShare but must remain in the selected plan for at least six months.

Standard Option

To participate in this option, members are not required to complete the requirements related to improving their health. However, members enrolling in the Standard Option who have a chronic medical condition are required to meet with a HealthShare Nurse Care Manager.  This option includes the same set of covered services, but with higher monthly costs and copayments than the Wellness Option.
